What type of information we have
When you do business with us, we will collect some or all of the following personal information from you: name; date of birth, postal address, email address and telephone number; financial and credit card information; GP’s contact details, medical history, consent forms of medical treatments (Botulinum Toxin, Dermal Fillers, Skin Boosters), signatures, before and after photographs of treatments; information about your computer or other electronic device, such as your IP address or Google analytics, and about your visits to, and use, of our website; information relating to transactions carried out between you and us, on or in relation to this website; and any other information that you choose to send to us. We may request additional information from you, for example when we conduct a survey or you enter a competition or prize draw. When additional information is requested, we will let you know at the time of collection how we intend to use it. We do we share customer details with any third parties.
How we get the information and why we have it
At various times during your interaction with our clinic you will be asked to provide personal information. If you contact us via the customer call centre, website, text or WhatsApp message, some or all personal data will be stored. After an appointment enquiry, you will receive the clinic’s Reopening policy, Covid-19 Questionnaire, and consent and medical history forms electronically via DocuSign. Forms are stored in the clinic's GoogleDrive under an encrypted name. We store personal details so we can contact you with information, such as newsletters, that you may find of interest regarding our services or of a general healthcare matter, or with requests for feedback. We would seek your consent prior to doing so, and you may subsequently withdraw your consent if you wish – simply let us know.
What we do with the information
We use your information held in the following ways: to maintain a database of our patients; to enable you to use the services available on the website; to process orders and supply you with products and services purchased via the website; to collect payments from you; to contact you or send you information directly relating to your order, booking, any survey or competition, or use of the website; to add you to relevant mailing lists and send you subscriptions, email notifications and/or newsletters which you have specifically requested; to send you marketing communications relating to anti wrinkle clinic, for example about our services, which we think may be of interest to you or where you have specifically agreed to this, by email or similar technology (but you can unsubscribe from these communications at any time; to deal with enquiries and complaints made by or about you relating to the website any products or services; For promotional and marketing purposes we may use a patient's before and after image. In this circumstance, we always crop the images so patient's identity does not show, in some cases blurring the eyes. We will always seek a patient’s consent before doing this. We will only use your personal data for the purposes for which we collected it, unless we reasonably consider that we need to use it for another reason and that reason is compatible with the original purpose. If we need to use your personal data for an unrelated purpose, we will notify you and we will explain the legal basis which allows us to do so. We will never pass on to patient information to third parties.
